What must I bring with me at
the time of application? A. If you are a sole proprietorship and are not using your own social security number in reporting business income, a copy of the Internal Revenue Service's Notice of Assigned Employer Identification Number. B. If you are a corporation or limited liability company, a copy of the Internal Revenue Service's Notice of Assigned Employer Identification Number and evidence of corporate existence (The State Certificate of Good Standing). Corporations should provide a filed copy of the Articles of Incorporation and Bylaws listing officers of the corporation. Limited liability companies should provide a filed copy of the Articles of Organization and Operating Agreement listing the name of the organizing member. All authorized signatories must provide an unexpired government-issued identification (for example a driver's license), and provide legal name, date of birth, physical address, and taxpayer identification (substitute identifications allowed for non-u.s. persons). If you are a partnership, non-profit organization, or a trustee of a formal trust, please contact a customer service representative for further details on required documentation.
